You must be a verified member of Unified Collective to access medical marijuana services offered by the collective. Membership at Unified Collective is free.

Please bring the following documents to your first visit:

  • Your ORIGINAL doctor’s recommendation.
  • A California issued Drivers License or Identification Card with photo. You must be at least 21 or older for membership.
  • Proof of California Residency (NO P.O. BOXES) – Utility Bills or Car Registration.

What happens during the verification process?

In order to verify your status as a qualified medical marijuana patient under California law, our staff will check the validity of your recommendation by contacting your referring physician. The verification process is usually quick, taking anywhere between 5 and 15 minutes.

After we receive verification from the doctor who wrote your recommendation for medical marijuana, we will make a copy of your recommendation and return the original to you.

As a verified member of Unified Collective, you will be able to access the collective’s services and additional website features. Your membership will be valid until the date your physician recommendation expires.

Please note that we must verify your recommendation before giving medicine each time you visit. In order to do these we will need to see your original doctor’s recommendation and photo identification card each time you pick up medication from Unified Collective.
